Building Occupancy Permit
(The intended workplace use must be specified for workplaces requiring a special building type.)
Photocopy of the Title Deed belonging to the workplace.
If the workplace is rented, a photocopy of the Lease Agreement.
Zoning Status Report.
Fire Department Report
(For business centers, general fire safety precautions are sufficient.)
Mastership Certificate
(Required for certain professions/businesses.)
Tax Certificate.
Chamber Registration and Trade Registry Record Copy.
Document Required for Workplaces Affecting Highway Traffic Safety.
If the license is to be issued in the name of a company:
- Photocopy of the Company’s Articles of Association
- Company Stamp/Seal
If the license is to be issued in the name of an individual:
- 2 Passport-sized Photographs
If the license is to be issued in the name of an individual:
- Photocopy of Identity Card
Consent of the Majority of Condominium Owners
(For sections registered as residential properties.)
Distance Measurement Report
(Required for coffeehouses, gaming halls, and places serving alcohol.)
Responsible Manager Document
(If required.)
Capacity Report
(If required.)
Measures and Arrangements for Disabled Individuals.
Environmental Impact Assessment (EIA/ÇED) Report
(If required.)
For Fuel Stations:
- Document indicating tank capacities.
Emission and Discharge Permits
(If required.)
If the license is to be issued in the name of an individual:
- Health Report
  (Obtained from the Tuberculosis Control Dispensary.)
If the license is to be issued in the name of an individual:
- Criminal Record Certificate
Work Permit for Foreign Nationals.
